Anyone who becomes a medical doctor must go through not only four years of college and pre-med courses, but also four years of medical school training and graduation.<\/span>\r\n\r\n<span data-contrast=\"auto\">For people to become Reproductive Endocrinologist and infertility specialists there are an additional four years of Obstetrics and Gynecology residency training. For individuals who want to sub specialize in the area of REI, there's an additional three years of subspecialty fellowship training and that involves clinical care of patients who have reproductive, endocrine and infertility needs as well as typically, a year of research during that three-year training. Once you finish all that and then pass all the specialty boards, you can call yourself a Reproductive Endocrinologist and infertility specialist.<\/span>\r\n\r\n<span data-contrast=\"auto\">The Reproductive Urology and infertility fellowship isn't officially recognized yet. But the RUs that are in our network are typically fellowship trained. And that means they've done (and this is a lot of work after their four years of med school), five to six years of general urology residency training. Then they do an additional one to two years of fellowship training in the area of male infertility and Andrology.<\/span>\r\n\r\n<b><span data-contrast=\"auto\">Dan Bulger:<\/span><\/b><span data-contrast=\"auto\"> Okay, so it's no easy task to become an REI or an RU. And perhaps that's why there are limited number of these sub-specialists in the U.S. About 1,300 Reproductive Endocrinologist and only about 300 to 350 or so, Reproductive Urologists.<\/span>\r\n\r\n<\/div>\r\n<span style=\"text-decoration: underline;\"><b>Why Visit an REI?<\/b><\/span>\r\n\r\n<span data-contrast=\"auto\">05:10 \u2013 06:10<\/span><span data-ccp-props=\"{}\">\u00a0<\/span><span data-ccp-props=\"{}\">\u00a0<\/span>\r\n<div style=\"padding-left: 40px;\"><b><span data-contrast=\"auto\">Dr. Janet Choi:<\/span><\/b><span data-contrast=\"auto\"> Whether it's because they come in the door saying, \u201clisten, I've been trying on my own to conceive without any luck for the past six months, a year, two years and I need help.\u201d Or people who are like, \u201cI'm not quite ready yet to think about having a child but I want to kind of prepare for the future because I know aging can affect things \u2014 so I want to talk to you about fertility preservation.\u201d While also taking care of patients who will say, \u201clisten, not interested in fertility, or family building at all but I do have very irregular periods. And one of my primary care doctors told me I had polycystic ovarian syndrome.\u201d So those are the different areas that an REI might assist you. REIs are also experts in the area of menopause therapy, a lot of us help women who are in their late 40s or 50s, manage and make their quality of life better, getting through the symptoms of hot flashes or mood swings or any other medical issues associated with that change.<\/span><\/div>\r\n<span style=\"text-decoration: underline;\"><b>Why Visit an RU?<\/b><\/span>\r\n\r\n<span data-contrast=\"auto\">06:28 \u2013 08:09<\/span>\r\n<div style=\"padding-left: 40px;\"><b><span data-contrast=\"auto\">Dr. Janet Choi:<\/span><\/b><span data-contrast=\"auto\"> Reproductive Urologists are the specialists you want to work with and see if you find that you're someone who has a low sperm count. An REI will typically have the male partner do a sperm analysis. That's one of the basic fertility tests. And if the sperm analysis is repeatedly abnormal, whether it's a super low amount of sperm, they're not swimming well, are abnormally shaped sperm, or some other things turn up; white blood cells can sometimes be a sign of an underlying infection for instance. They\u2019ll say, \u201cWhy don\u2019t you meet with a Reproductive Urologist.\u201d So, Reproductive Urologists not only take care of male fertility needs, whether it's due to sperm issues. But other needs like testicular cancer and prostate cancer needs, they can sort of pick them up sometimes at first sign of another medical issue. Like for instance, diabetes, or testicular cancer might be an abnormal sperm analysis. Reproductive Urologists can also help you with issues relating to low testosterone. Lastly, they can also help with sexual function issues. If it comes to issues with erectile dysfunction, ejaculatory dysfunction or other forms of sexual dysfunction. They can help support you in that area as well.<\/span><\/div>\r\n<span style=\"text-decoration: underline;\"><b>When to See Both<\/b><\/span>\r\n\r\n<span data-contrast=\"auto\">08:09 \u2013 15:00<\/span>\r\n<div style=\"padding-left: 40px;\">\r\n\r\n<b><span data-contrast=\"auto\">Dan Bulger:<\/span><\/b><span data-contrast=\"auto\"> REIs and RUs tend to work hand-in-hand for their patients when there is male infertility present.\u00a0 Couples who have a problem with sperm sample results utilize ICSI, a game changing resource whereby a single sperm can be injected directly into an egg. And with ICSI, success rates for those with male-factor infertility are extremely good. So, if ICSI and IVF are great tools for those with male-factor. Then why would one need to see and RU?<\/span>\r\n\r\n<b><span data-contrast=\"auto\">Dr. Janet Choi:<\/span><\/b><span data-contrast=\"auto\"> I'm pretty quick to send a male patient to Reproductive Urologist. So, if I see low sperm counts, like I said earlier, any kind of analysis, I will tell the patient I think it's prudent and hopefully in your best interest to go. There is a real benefit to men in seeing an RU when trying to conceive. And there's a real case to be made that these men should probably get themselves evaluated by an RU, while a female partner is being evaluated by an REI. Any couple where there's a male fertility issues \u2014 those males would benefit from getting at least a quick evaluation with a Reproductive Urologist. And lo and behold sometimes they will evaluate the male patient and find something that's treatable and fixable. Something that improves the sperm quality or helps improve the odds of a pregnancy resulting, because that's what you want. And along the way, find some health issues that could have been a major problem, like, let's say a small testicular nodule that they pick up early on and is then more treatable.
